Houseless Outreach Stabilization and Transition (HOST)
Program
The HOST Program is designed to reach people whose severe and disabling substance use and behavioral health symptoms keep them from being able to receive conventional physical and behavioral health services.

What is the eligibility criteria for HOST?
HOST Eligibility Criteria:

Adult (18 or older)
-
Unhoused or imminent risk of losing housing
-
Severe substance use or co-occurring mental health disorder
-
Unable or unwilling to access traditional behavioral health services.
