Houseless Outreach Stabilization and Transition (HOST)
Program

A team of behavioral & physical health practitioners providing critical survival services to individuals experiencing severe & persistent behavioral health disorders in addition to experiencing homelessness. 

What is the HOSTEligibility criteria?

  • Adult (18 or older) 

  • Unhoused or imminent risk of losing housing

  • Substance use or co-occurring behavioral health disorder

  • Unable or unwilling to access traditional behavioral health services

What servicesare avaliable?

  • Substance Use Assessment and Treatment

  • Mental Health Assessment and Treatment

  • Intensive Case Management

  • Peer Counseling

  • Street Medical Outreach

  • Psychiatric Evaluation and Medication Management
